- HDR/PCN — PCN varies per plan sponsor — pull from member card
- AM04/AM04.302 — Cardholder ID (302-C2)
- AM07/AM07.436 — Product/Service ID = NDC-11 (436-E1 qualifier '03')
Fixed values
- HDR.102 = D0 — Version/Release always 'D0' for D.0
- HDR.103 = B1 — Transaction Code fixed for billing
